Out-of-Network Reimbursement
A Note on Privacy & Coverage:
AQZA Mental Health Haven provides all standard diagnostic codes, CPT codes, provider credentials, and tax identifiers required for reimbursement. Final eligibility and reimbursement rates are determined solely by your individual insurance plan terms.
